Why did insurance deny my crown, and do I still have to pay?
This question fills entire forums. A dentist says the crown is necessary, the insurance company says no, and a bill shows up that nobody warned you about. Here is what's usually going on.
Dental insurance doesn't decide whether you need treatment. It decides what your plan pays for, and those are two different things. Most crown denials come down to fine print. Many plans only cover a crown once damage crosses a threshold written into the policy, often more than half the tooth. Most won't pay to replace a crown they already paid for until a set number of years has passed. And plenty of claims get denied simply because they went in without the X-rays and notes the reviewer wanted to see, which the letter calls "lack of documentation." None of that means the crown wasn't needed. It means the policy and the paperwork didn't line up.
Do you still owe the bill? Usually, yes. The treatment already happened, and your policy is a contract between you and your insurer. That's the part that stings, because by the time the denial letter arrives, it's too late to make a different decision.
