1. Online, Before Your Visit (Easiest)
When you reserve your appointment, you’ll receive a text and email from our secure patient portal, Oryx. Create your account and complete your forms right from your phone or computer, on your couch, on your schedule. Done before you walk in.

Prefer to do it here? Just arrive about 15 minutes early and we’ll hand you an iPad at the front desk. Our team is right there if you have questions.
3. On Paper, If You’d Rather
No problem at all. Let us know and we’ll print the medical and dental history forms for you when you arrive.
Why Our Health Forms Are More Thorough
Our questionnaires go deeper than the average dental office’s, on purpose. We use forms developed through the Kois Center, the advanced-training institute where Dr. Abboud studied, because your mouth doesn’t exist in isolation. Your medications, health history, sleep, and risk factors all shape the right dental plan for you. A few extra questions up front is how we find causes instead of chasing symptoms.
